Projects for Assistance in Transition from Homelessness
P.A.T.H. is an outreach program for adults who are homeless and have a mental illness. The purpose of the program is to locate and engage the homeless, chronically mentally ill individuals and provide case management, transitional housing, and referral services to assist them in achieving a higher level of functioning as well as a greater degree of independent living.
Outreach work is a key function of the staff. The staff also provide life skills training, medication management, individual counseling, and a support group to assist these individuals in stabilizing and transitioning to independent living or group homes—all this based upon their level of functioning.
P.A.T.H. was one of seven programs nationwide that received the Exemplary Program Award by the center for Mental Health Services in Washington, D.C.
